Recognize the setting in which infection develops with chromosomal Beta-lactamase
producing bacteria.
Explanation:
Beta lactamase production is nowadays commonly encountered but more so in hospital settings than in the out-patient.
It is more common in patients already on a beta lactam antibiotic e.g. drug abuser who takes occasional antibiotics.
It should be treated - as everybody knows - with Vancomycin in case of Staph and Strept. but with those bacteria whose names end with bacter-e.g. Enterobacter or citrobacter -use the big guns like Imipenem or Meropenem.
