There is no established maintenance therapy. Steroids don't work in maintenance. 5-ASA compounds have had mixed results. Recent data suggests 6MP and Methotrexate can be useful for maintenance in severe cases.

The role of steroids is there in suppressing acute flares but not in maintenance. Remicade is helpful in those with fistulae and must be started only after PPD testing is performed as 2% of people on Remicade will develop TB.
