A 54 year old patient came to the clinic with pain in the
perineal area. He noticed swelling in the area 2 days ago and it had worsened. He was
afebrile but the odor in the room was putrid. The scrotal area findings are shown in the
picture.
What is your diagnosis (give the specific name)
Answer: Fournier's Gangrene
This picture is typical of Fournier's gangrene. It develops in the groin area and in this patient it came on over a few hours, developing from swelling to necrosis. The sampling grew mixed flora. Patient underwent debridement, which is the best treatment for this condition. Debridement may leave the testes hanging loose but typically, it does not involve the testes themselves. If this were a peri-anal abscess, one would see erythema and swelling but not necrotic gangrenous tissue like in this picture.
