This patient has DKA. There is a metabolic acidosis picture with increased anion gap.
In DKA, the main etiology is that the body lacks insulin. Instead of breaking down glucose for energy requirements, it thus breaks down Fatty acids to fulfill its energy requirements. This leads to the production of Ketones.
We need to promote glucose metabolism in order to prevent fatty acid metabolism. For this, more insulin is required as evidenced by the anion gap acidosis that has not closed off even after the insulin has been given for two hours and the glucose level is down. In order to prevent hypoglycemia while we infuse this extra insulin, we have to provide glucose externally and carry on with the insulin.
